Join our team today! The Hospice RN Case Manager is accountable for the delivery of quality home care services to the program's patients in conjunction with physician's directions, company and industry standards, and insurance guidelines as assigned.
POSITION REQUIREMENTS:
A.
Minimal Education:
1.
Graduate of accredited school of nursing.
2.
Bachelor's degree of Science in Nursing is preferred.
B.
Minimal
Experience:
1.
Minimum of one (1) year nursing experience.
2.
One (1) year skilled nursing facility experience preferred.
C.
Other
Qualifications:
1.
Current Nursing License.
2.
CPR certified.
3.
Ability to function with minimal supervision.
4.
Excellent communication, organizational and human relation skills.
ESSENTIAL DUTIES AND
Responsibilities:
1.
Follows established Standard of Nursing Practices and implement Facility policies and procedures.
2.
Knowledgeably and competently performs nursing skills.
3.
Identifies safety hazards and initiates corrective action immediately including assuring the safety of Residents under his/her supervision.
4.
Interpret existing Policies and Procedures to Nursing Assistants, Restorative Nursing Assistants, Residents, Families and Physicians.
5.
Upon admission and on an ongoing basis, assesses the patient's physical and emotional status, level of competency/knowledge, and safety.
6.
Identifies health and safety hazards and initiates corrective action immediately including assuring the safety of Residents under his/her supervision.
7.
Supervises and evaluates all direct resident care and initiates corrective action as necessary.
8.
Receives and provides report from/to nurse he/she is relieving/coming on duty and records sufficient information to implement appropriate follow-up as necessary.
9.
Develops an accurate comprehensive care plan based on each resident's assessment.
10.
Provides direct resident care and gives instruction to Nursing Assistants.
11.
Consistently works cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies; and ancillary service providers.
12.
Formulates and periodically updates care plan as necessary and as required by Regulations.
13.
Listens to resident, family or physician concerns and initiates corrective action and/or reports concerns to RN Supervisor.
14.
Provides clinical supervision of Nursing Assistants providing direct resident care.
15.
Assesses resident's needs and initiates development of individualized plans of resident care.
16.
Participates in orientation of new employees to facility policies and procedures and processes.
17.
Participates in interdisciplinary case conference and discharge planning.
18.
Records and reports changes in the patient's condition to physician and RN Supervisor in a timely manner.
19.
Documents all orders in accordance with regulatory and facility requirements.
20.
Delivers care according to physician orders, plan of care and facility standards.
21.
Orders, prepares and administers medications, enteral and parenteral nutrition and treatments as ordered.
22.
Maintains current technical and clinical skills, certifications and licensing.

23.
Attends and participates in in-services, clinical record review process, conferences, meetings and facility Continuous Quality Improvement activities.
24.
Performs other duties as assigned by supervisor or manager.
25.
Follows standard precautions and infection control procedures.
26.
Lifts residents and transports during fire/disaster drills and in actual emergencies.
May respond to emergencies outside unit or campus as needed.
27.
Maintains strict confidentiality of resident, employee and business information.
28.
Provides for a safe environment for residents, physicians and employees.
29.
Meets with Residents and residents' families/representatives to resolve resident concerns.
30.
Participates in orientation of new employees and evaluations of CNAs and LPNs.
31 Performs other duties as assigned by supervisor.
